Critical Roles

In ADH's Hospice service, social workers fill multidimensional roles--as clinicians, educators, advocates, and community leaders. They provide social services and counseling to patients and families and arrange for necessary ministerial support and other volunteer bereavement counseling. They:

  • Assess patient and family psychosocial response to terminal illness
  • Evaluate coping skills in crisis situations
  • Help develop the patient's Plan of Care
  • Participate in Interdisciplinary Group meetings
  • Provide counseling services
  • Assist patient and family in using community resources and make referrals as necessary
  • Evaluate the family's financial situation
  • Coordinate transfer and discharge planning
  • Participate in the training of staff and volunteers
  • Consult with the Volunteer Bereavement Program
  • Help with ongoing quality assurance
  • May assist with ministerial support and psychosocial activities
  • Act as consultants to other Hospice personnel
  • May provide bereavement counseling
  • Assist Volunteer Coordinator in organizing volunteer bereavement counseling network

Qualifications

  • Must be a graduate of an accredited Program of Social Work
  • Must be licensed within the provisional period
  • Should be team-oriented and able to work collaboratively with others in the homecare/hospice team.
