Critical Roles
In ADH's Hospice service, social workers fill multidimensional roles--as clinicians, educators, advocates, and community leaders. They provide social services and counseling to patients and families and arrange for necessary ministerial support and other volunteer bereavement counseling. They:
- Assess patient and family psychosocial response to terminal illness
- Evaluate coping skills in crisis situations
- Help develop the patient's Plan of Care
- Participate in Interdisciplinary Group meetings
- Provide counseling services
- Assist patient and family in using community resources and make referrals as necessary
- Evaluate the family's financial situation
- Coordinate transfer and discharge planning
- Participate in the training of staff and volunteers
- Consult with the Volunteer Bereavement Program
- Help with ongoing quality assurance
- May assist with ministerial support and psychosocial activities
- Act as consultants to other Hospice personnel
- May provide bereavement counseling
- Assist Volunteer Coordinator in organizing volunteer bereavement counseling network

Qualifications
- Must be a graduate of an accredited Program of Social Work
- Must be licensed within the provisional period
- Should be team-oriented and able to work collaboratively with others in the homecare/hospice team.
